About the Role

Summary/Objective The LMHP/MHP supports the solution-focused interventions intended to achieve identified goals or objectives as set forth in the individuals individualized treatment plan. In addition, the LMHP/MHP provides skill training through psycho-education and strategies and develops specific behavioral change plans for patients and behavioral health protocols for target populations.

Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Assists primary care team in recognizing and treating mental disorders and psychosocial problems.

Assesses the clinical status of patients referred by primary care providers through brief consultative contacts.

Works with the primary care team to efficiently and effectively treat and manage patients with chronic emotional or health problems.

Works with the care team to refer cases to mental health specialists as appropriate.

Assists in the detection of at-risk patients and development of plans to prevent further psychological or physical deterioration.

Assists in preventing relapse or morbidity in conditions that tend to recur over time.

Evaluates patient care plans with primary care team.

Teaches patients, families and staff about care, prevention and treatment enhancement techniques.

Monitors the sites behavioral health program, identifying problems related to patient services and making recommendations for improvement.

Attends and participates in meetings and quality improvement activities as required.

Serves as a member of committees as requested.

Participates in evaluation of peers and support staff.

Furthers the mission of the organization through active support of the strategic goals.

Requirements

Supervisory Responsibility: This position has no supervisory responsibilities.


Work Environment: While performing the duties of this position, the employee travels by automobile and is exposed to changing weather conditions. The employee works in a variety of locations, including an emergency room or clinic setting, in addition to other community locations where the person lives, works, attends school and/or socializes based on assigned caseload.


Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job, the employee is required to speak and hear sufficiently to communicate effectively by phone or in person at normal volumes. The employees vision must be adequate to read correspondence, computer screen, forms, etc. The position requires good manual dexterity.


Position Type and Expected Hours of Work

This is a full-time position. Expected hours are 40 hours per week, Monday through Friday.

This position is a contract fee for service position upon caseload availability.


Travel: Occasionally


Required Education and Experience

  • Practitioners with a masters degree in social work, counseling, psychology or a related human services field may provide all aspects of CPST, including counseling

  • Bachelors degree in social work, counseling, psychology or a related human services field or four years of equivalent education and/or experience working in the human services field. Can include certified peer support specialists who meet the qualifications above.

  • Certification in the State of Louisiana to provide the service, which includes criminal, professional background checks and completion of a State-approved, standardized basic training program.


Additional Eligibility Qualifications

  • Current state licensure as a licensed clinical social worker, licensed clinical professional counselor or licensed clinical psychologist.

  • Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.

  • Ability to make quick and accurate clinical assessments of mental and behavioral conditions.

  • High level of comfort in working with primary care medical providers using a consultative style.

  • Ability to work through brief patient contacts, including via telephone and teleconferencing.

  • Good knowledge of psychopharmacology.

  • Ability to design and implement clinical pathways and protocols for treatment of selected mental and behavioral conditions.

  • Ability to design effective psycho-education curricula and lead effective psycho-education classes.

  • Ability to work effectively as a team, interacting with patients, primary care team, consulting psychiatrist and mental health specialists, as well as with administrative and support staff.

  • Ability to exercise balanced judgment in evaluating situations and making decisions, and to handle difficult or confrontational situations in a calm, consistent and equitable manner.

  • Ability to effectively represent company’s interests in the community and maintain effective working relationships among co-workers and public, private and professional groups.

  • Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures or governmental regulations.

  • Experience working with limited resource population.
